Titans Season 2 Temporarily Shuts Down Production After Stunt Coordinator Died During Test Run

Special Effects Coordinator Warren Appleby was struck by a heavy object during a stunt test and passed away as a result. This was done at a test facility where the crew would enact out scenes to try them out before the actors executed them.

Canada’s Ministry of Labour is investigating the case. The stunt involved a car flipping over and according to Comicbook.com, a piece of shrapnel struck Mr. Appleby in the head. He died on route to the hospital.
